Admission requirements:
- Self-advising Worksheet.
- High school graduate or GED.
- Health statement completed.
- Ten hours of job shadowing and submission of the Job Shadow Information form.
- A current Healthcare Provider CPR card (contact Program Chair for specific requirements) is required prior to PTAS1301.
- All courses must be passed with a minimum grade of C+ to progress in the program, and all PTAS classes must be taken in sequence.
- A criminal background check will be required of each student in this program. Based on the outcome of the background check, a student may be prevented from taking certain courses, accessing certain laboratory experiences, or completing the program. A non-refundable fee of $45 will be assessed for this CBC.
- Computer skills in keyboarding and windows will be helpful.

- All students are required to have liability insurance. Cost of this insurance is included in the student fees at the beginning of each quarter that includes clinical education.
